Ares Men’s Health (“Ares Men’s Health,” “we,” “us,” or “our”) provides telemedicine-based healthcare services through secure electronic communication platforms. These services allow patients to consult with licensed healthcare providers remotely using audio, video, and other telecommunications technologies.
By using the telehealth services offered through Ares Men’s Health (the “Services”), you acknowledge and agree that your medical care may be delivered through telehealth technology rather than through an in-person visit.
Telehealth involves the delivery of healthcare services using electronic communications between a patient and a licensed healthcare provider who are not in the same physical location. Telehealth may be used for patient evaluation, diagnosis, consultation, treatment, follow-up care, prescribing medications when appropriate, and patient education.
Telehealth services may include, but are not limited to:

Electronic transmission of medical records, photographs, personal health information, or other medical data between you and your healthcare provider

If your provider determines that telehealth is not appropriate for your condition, you may be advised to seek in-person medical care or emergency treatment.
Patient Acknowledgments and Consent
By selecting “I Agree,” checking a consent box, or otherwise continuing to use the Services, you acknowledge and agree to the following:

I understand that telehealth services are not intended for emergency medical conditions.

If I believe I am experiencing a medical emergency, I will call 911 or seek immediate in-person medical care.

I voluntarily consent to receive healthcare services through telehealth technologies from providers associated with Ares Men’s Health.

I understand that telehealth is an evolving field and that the technologies used in my care may change over time.

I understand that technical failures may interrupt my consultation or result in delays in care.

I understand that no specific medical outcome, result, or improvement can be guaranteed.

I understand that my condition may not improve and could potentially worsen despite treatment.

I agree to provide accurate, truthful, and complete information regarding my medical history, medications, symptoms, and health status.

I understand that providing inaccurate or incomplete information may result in improper diagnosis or treatment and could potentially result in harm.

I understand that my healthcare provider may determine that my condition is not appropriate for telehealth treatment and may recommend an in-person evaluation.

I understand that telehealth services are intended to supplement, not replace, relationships with other healthcare providers when applicable.

Privacy and Use of Health Information
I understand that my personal information and protected health information will be collected, used, and protected in accordance with the Ares Men’s Health Privacy Policy and applicable healthcare privacy laws.
My healthcare information may be used for purposes including:

Medical treatment and care coordination

Prescription management and pharmacy communication

Healthcare operations and administrative purposes

Compliance with applicable legal and regulatory requirements
My medical records may be shared with other healthcare providers only with my authorization or as permitted by law.
Withdrawal of Consent
I understand that I may withdraw my consent to receive telehealth services at any time by notifying Ares Men’s Health in writing. Withdrawal of consent will not affect services already provided but may limit my ability to receive future care through telehealth.
Unless withdrawn, this consent will remain in effect and will apply to all future telehealth encounters with Ares Men’s Health providers.
